Hearts for Dementia Home care is one of the only home care agencies in Arizona led and operated by a dual board-certified Family and Psychiatric Mental Health Nurse Practitioner. We develop person-centered, customizable care plans that take into consideration your medical and mental health needs. Our support encompasses your physical, social and emotional well-being.
We believe in a Team Approach
At Hearts for Dementia Home Care, we have the ability to collaborate and coordinate care with your health care team. Our first-class home care staff believe in providing high-quality care. We can work directly with your health care provider to offer the very best in-home care support. Working with your health care provider allows us to decrease potential hospitalizations and injuries. Our Nurse Practitioner or Registered Nurse, will develop a comprehensive care plan giving careful thought to your emotional, physical and mental health. All Nurses and Personal Care Attendant’s receive clinical oversight by our Nurse Practitioner. Our Personal Care Attendant’s have the ability to consult with the Clinical Director when necessary.
All Personal Care Attendant’s receive extensive Dementia Care training and use the Montessori philosophy to help your loved one stay engaged.
Caregiver Training
Hearts For Dementia Home Care provides ongoing training to ensure the latest evidenced based practices in dementia care are exercised to benefit you or your loved one.
All Personal Care Attendants will receive training by The National Council for Certified Dementia Practitioners (NCCDP). NCCDP is a global leader and pioneer in dementia education and certifications.
All Hearts For Dementia Home Care Personal Care Attendant’s complete 8 hours of instructor led training pre-employment, including the NCCDP Alzheimer’s Disease and Dementia Care Seminar led by a certified Alzheimer’s support group. Personal Care Attendant’s are required to have a minimum of 8 hours of ongoing training per year. The NCCDP curriculum is recognized by the Centers for Medicare and Medicaid Services, universities, and healthcare organizations across the country.
Montessori-Based Dementia Programming
The Montessori philosophy allows your loved one to stay engaged in activities with meaning and purpose based on interests prior to their dementia. Montessori-based activities uses a person-centered approach to keep them engaged in meaningful activities that will help enhance participation and enjoyment. Engaging in purposeful activities activate the mind by using the sensory, motor, cognitive, and social aspects of the brain. Personal Care Attendant’s will implement the Montessori Method for Dementia throughout the day as part of your loved one’s care plan. Using this philosophy, we aspire to bring smiles back into their life by empowering your loved ones to live with purpose.
Support Groups
We at Hearts for Dementia Home Care understand the importance of feeling supported. Our facilitated support groups will bring people together with similar life experiences. People can share their personal experiences and coping strategies. We aim to provide a warm and friendly atmosphere where members feel safe, respected and are encouraged to speak about their lives freely and openly. This will be a safe place to encourage members to share stories and information and offer empathic validation and support. An educational component may be added to the structure of the session.
